When the subscriber receives medical care, the medical claim is first submitted to the primary insurance company for payment (Vines et al., 2017). After the primary plan pays part of its medical expenses, if the secondary plan is purchased, it will pay the remaining medical costs (Vines et al., 2017). Examples of primary insurance are employer provided health insurance and Medicare (Vines et al., 2017). Upon retirement Medicare becomes the primary provider and employer coverage is secondary (Vines et al., 2017). Secondary insurance coverage is a medical coverage plan purchased separately from the primary plan (Vines et al., 2017). Dental, vision, and accidental injuries are examples of secondary programs (Vines et al., 2017). The secondary plan is considered a type of personal insurance that provides additional protection after the primary insurance processes the claim (Vines et al., 2017). When individuals have both primary and secondary insurance plans they will usually pay for two deductibles and two premiums (Vines et al., 2017).
Original Medicare covers only a portion of healthcare costs leaving the remainder such as copays and deductibles for the subscriber to pay (Vines et al., 2017). If an individual is not retirement age and has a primary insurance provider the claim is then submitted first to the primary provider followed by submission to Medicare (Vines et al., 2017). Cost sharing is when an individual pays out of pocket costs such as coinsurance, co-payments, and deductible, which is the portion of the payment not covered by the insurance provider (Vines et al., 2017). The out-of-pocket expenses will vary per provider (Vines et al., 2017). The coordination of insurance benefits determines the payment responsibilities covered by other contribution plans (such as secondary and supplementary insurance) and the expected primary provider payments and services (Vines et al., 2017).
Supplemental insurance such as life insurance, long-term insurance, and Medigap insurance, which provides additional coverage for costs not covered by primary insurance or Medicare (Medicare, 2021). The cost of Medigap varies by insurance carrier and state supplemental insurance is not a stand-alone insurance like secondary insurance (Medicare, 2021). Examples of supplementary insurance include long-term care insurance, which is used to pay for chronic diseases or disabilities that are not covered by primary insurance (Medicare, 2021)

This leads to increased length of stay (LOS) in which higher costs are incurred. Each individual episode of hospitalization has a strategically determined average length of stay (ALOS) (Vines et al., 2017).
